  • Circumstances of loss: In number 3, write how, when, why, and where your SIM was lost.
  • Purpose of Affidavit: Write the purpose of your request for an Affidavit of Loss document in number 5.
  • Declare: In number 4, declare that the information given is correct.
  • Signed and Dated: I signed and dated the document at the end.
