You can get a lost SIM card affidavit letter online, by visiting a notary public, or you can write it down. So, follow the simple steps.
Get the affidavit online:
- Go to the official website of Freedom of Information Philippines.
- Here, you can send an application to get an affidavit. So, click the Create a Request button on the top right.
- Next, log in to your account. Send them all the information about your lost Globe SIM card, such as the owner’s name, nationality, address, customer signature, mobile number, and incident details (why, how, when, and where the mobile phone or SIM was lost).
- Submit your application and wait for the process to begin.
- The Globe customer service will reply to you soon, and then you need to follow the instructions that will be given to you for further processing.
Visit a Notary Public:
- Visit the nearest Notary Public Office.
- Inform them all about why, when, where, and how your SIM is lost.
- Provide them with all SIM details, such as the owner’s name, nationality, address, customer signature, and mobile number.
- Also, give them 2 originals and photocopies of ID cards.
- They will issue an affidavit document for the lost SIM Card.

- Address Details: First, on the left side, you enter your address details, such as country and city.
- Title: Affidavit of Loss, Write the title.
- Identification: Enter your name, age, marital status, and address in number one.
- Lost property: In number two, write the number of the SIM card that you have lost.
- Circumstances of loss: In number 3, write how, when, why, and where your SIM was lost.
- Purpose of Affidavit: Write the purpose of your request for an Affidavit of Loss document in number 5.
- Declare: In number 4, declare that the information given is correct.
- Signed and Dated: I signed and dated the document at the end.
- Notarized: Go to the public attorney’s office to have them verify and sign the information you provided that it is legal.
